Claim Information

To submit a claim use one of the appropriate links below. If none are available for your particular claim, please contact us directly.

Insurance is intended to protect you in the event of a loss.  When you have a covered claim, your insurance policy is designed to put you back to where you were when the loss occurred.  The guiding principal of insurance claims resolution is indemnification.  Here are some things you should do if one of the following losses occurs:

AUTO INSURANCE CLAIMS
:
1. Make sure the police are notified of the accident so that an official Police Report can be generated, and to allow action by appropriate emergency responders.
2. Make sure all drivers involved have the contact and insurance information for all other drivers. 
3. Call our office to notify us and to ask any questions.
4. Depending on who was "At Fault" we may direct you to call the Claims Department of your insurance company.  We'll give you the phone number when you call, but it can also be found on the Insurance ID Card that is kept in the vehicle.  The claims department will need to hear what happened directly from you.

HOME INSURANCE AND PROPERTY INSURANCE CLAIMS
:
1. Notify appropriate authorities (police, fire department, etc) of the loss. 
2. Call our office to notify us and to ask any questions.  We'll guide you to the next step.
3. We will usually direct you to call the Claims Department of your insurance company.  We'll give you the phone number when you call, but it can also be found in your policy documentation.  The claims department will need to hear what happened directly from you.


HEALTH INSURANCE CLAIMS
:
-Usually the doctor/provider's office will take care of getting the claim resolved and proper payments applied.
-If there is an issue that you need help with, let us know and we can usually work directly with the provider and the insurance company to get the issue resolved.
-It's always a good idea to get all procedures and operations pre-authorized to ensure that it is a procedure the insurance policy covers.  This can be done the easiest through the provider's office (meaning doctor's office), but you can also call Member Services whose phone number can be found on your insurance card, or call our office and we can help as well.  The provider will need to supply the appropriate billing or procedure code.


LIABILITY INSURANCE CLAIMS
:
Notify our office immediately.  We will discuss the potential claim and help you decide what the next step will be, and when to notify the insurance company.
